Description

Coordinated care within the care management model of service delivery is widely recognized as a best practice for patients with chronic medical conditions. The online care management curriculum, is geared to engage learners through quizzes, videos and real-life scenarios that can be transferred to daily practices. Participants will learn to serve as productive members of interdisciplinary care teams who can efficiently oversee patients’ wellness needs, help set patients’ goals, and educate patients to make sure they adhere to their care. Additionally, the series helps front line staff establish strong positive relationships with patients and empowers them with the information needed to connect patients to needed resources within their community or healthcare system.

Additional modules and resources are added continuously and participants are advised to regularly check back for new content.
