Once you have registered for the event, we ask that you pick from one of the following breakout sessions by filling out the form below. The breakout sessions are as follows:
1. Managing Navigation & Care Management Case Loads: Facilitated by Alanna Laino
This breakout is intended for care navigators, program leads and supervisors. The group will explore successful models and discuss ways to manage screening targets and care management caseloads.
2. Enhanced Services Provider Form: Facilitated by Ashley Restaino
This breakout is intended for referral coordinators and enhanced services providers. The group will identify areas for improved workflow, improved coordination between organizations, and needs for better access to information and reporting. Members from Ready Computing will be joining this breakout session.
3. Sustainability & Demonstrating Value: Facilitated by Joe Conte
This breakout is intended for leadership and finance staff. The group will discuss needs for sustainability, potential models, and how to develop an equation to prove impact and value of the NYHER waiver services.
