3.g.ii Integration of Palliative Care into Nursing Homes, Module Six: The Conversation and Communication for End-of-Life

This module explains how communication is an essential competency in palliative care and identifies barriers and myths that prevent effective communication. It describes factors that influence communication in dealing with older adults and discusses older adult and family expectations when communicating with the interdisciplinary team. Effective palliative care communication interventions and guidelines, including active listening, mindful presence, facilitating family meetings, breaking bad news, end-of-life discussions, team communication and resolving conflicts are all reviewed. Interdisciplinary staff is shown how to utilize Advanced Care Planning (ACP) Decisions video program for residents and family members, either in the SNF or at home for family.
