Member Complaints

At any point, a Member has a right to report a complaint to their health insurance plan, a provider, or a Social Care Network. 

You can report a complaint to the Social Care Network by completing the form below. Your complaint will be reviewed in a timely manner. You will receive a written response with the action taken or resolution within 30 days of your submission.
