Social Determinants of Health & Health Equity Programs
Social determinants of health (SDOH) are the conditions in the environments where people are born, live, learn, work, play, worship, and age that affect a wide range of health, functioning, and quality-of-life outcomes and risks. SDOH also contribute to wide health disparities and inequities, raising the risk of chronic health conditions.

Staten Island PPS is partnered with community-based organizations including faith-based agencies to engage people in their neighborhoods around various public health topics. Many community health initiatives and programs focus on social determinants of health, special populations, and health equity. All programs are designed with input and participation from community agencies and their members, with an underlying focus on health equity.
