Executives and program leaders of community health, faith-based, and healthcare organizations are invited to join the Staten Island PPS as we plan for a potential new Medicaid 1115 Waiver program and develop future state population health initiatives.
Hear how community health agencies are partnering to improve population health, positioning their organizations for value-based navigation contracts, and sharing successes and challenges. Attendees are also invited to share feedback and potential focus areas for 2023 and beyond.
Amir Bassiri, New York State Medicaid Director, will give a keynote presentation at 11am describing the waiver and the current state of health in New York.
