NYS Medicaid 1115 Waiver: Health Equity Reform

Social Care Network (SCN)

In January 2024, the New York State Department of Health (NYS DOH) announced it’s newly approved Medicaid 1115 waiver amendment, New York Health Equity Reform (NYHER): Making Targeted, Evidence-Based Investments to Address Health Disparities Exacerbated by the COVID-19 Pandemic. Through the waiver amendment, NYS aims to coordinate regional social care service delivery, improve health equity and health outcomes, and enhance integration with physical and behavioral health. NYS will set out to establish Social Care Networks (SCN) to serve as regionally based organizations responsible for building networks of CBOs and other providers to better enable health-related social needs (HRSN) screening and the delivery of social care services by CBOs.

NYS DOH released a request for applications (RFA) in late January seeking 13 entities to serve as lead Social Care Networks (SCN) across New York State. In April, Staten Island PPS submitted an application to serve as an SCN lead entity in Richmond County. As we await the notice of awards from NYS DOH, Staten Island PPS continues to engage its existing network of partners and build new relationships as we prepare for this future state.
