Diabetes Self-Management Program at SIUH
The Diabetes Self-Management Program at SIUH is nationally recognized by the American Diabetes Association. The program consists of Certified Diabetes Care and Education Specialists who can help people with all types of diabetes better manage their blood glucose. The program offers individualized counseling sessions to address the day-to-day challenges of living with diabetes.

Diabetes Center at Health + Hospitals/Gotham Health
Services on-site include eye and foot care, virtual nutrition counseling, and the use of innovative technology to monitor blood sugar. Additionally, Richmond University Medical Center has partnered with the clinic to provide vascular screenings on site. They will provide referrals to the hospital for vascular surgery evaluation and treatment.
