HealthFirst Navigation

Studies show many factors that happen outside the doctor’s office can impact the overall health and wellness of individuals. Staten Island PPS provides care navigation and linkage to Primary Care and social determinants of health services to individuals with Hypertension, Diabetes, Obesity, HIV, and other chronic conditions. Care navigators also assist with helping members schedule annual screenings. Our care navigators conduct outreach, complete a Patient Perception of Health (assessment), provide support, and connect patients to community-based resources as needed. Staten Island PPS care navigators and partner organization navigators use a proprietary platform to document and make automatic closed loop referrals.

The goal of care navigation is to help Staten Islanders improve their overall health and wellness.
