Call 1-800-772-1213 from 7 am to 7 pm to make an in-person appointment OR visit www.ssa.gov/disability to review the disability checklist to know what you need to begin the process. Complete the Benefit Application for eligibility and payment information. Complete the Disability Report to provide work and medical information. Fill out the Authorization to disclose information and either mail it to the Social Security Administration or bring it to your Social Security office.
