John worked as a Project Manager under the SI PPS umbrella for Community Health Action for the past 9 years. He was responsible for CHASI’s Food Insecurity Project, Veteran’s Project, Behavioral Health Project & Social Determinants of Health Project. Under DSRIP, he managed CHASI’s Patient Activation Management Project, Substance Abuse Projects, Harm Reduction Project, Health Literacy Project & their CBC’s Project.
John had joined SI PPS as a Field Operations Manager, responsible for coordinating the Channels onboarding and training for partners joining the SCN as well as coordinating with the screening and navigation team.
