Ride Health

Ride Health partners with healthcare organizations and transportation providers to coordinate transportation to approved locations as part of the New York Health Equity Reform Waiver program through the Staten Island Social Care Network.

This service supports PLANNED, COORDINATED and SCHEDULED rides related to health-related social needs activities. To avoid delays, rides should be scheduled well in advance before a needed appointment. THIS PROGRAM DOES NOT SUPPORT LAST MINUTE TRIPS. LAST MINUTE OR SPUR OF THE MOMENT TRIPS WILL NOT BE APPROVED.
If members call outside normal business hours, a call will be routed to a general Ride Health coordinator. The general 24/7 Ride Health support line can only assist with EXISTING rides. They will not be able to schedule new, last minute, or spur of the moment rides.

Eligible locations include:
housing appointments, housing court, court appointments, probation or parole appointments, local department of social services, department of motor vehicles, classes for education or support for chronic conditions, childcare (for health related social services ONLY), parenting classes, food pantries, mobile markets or other location where member is receiving food related assistance

Rides are not eligible for job training, transportation to a job, or other employment related reasons. The service cannot be used for running errands, pharmacies, medical appointments, or grocery stores.
Rides cannot be used to drop children off at childcare for any reason except when the member needs transportation to one of the locations above.

There is a $1,000 member cap for the program period on rides. Once a member meets the cap, they are no longer eligible for rides.

A coordinator from Ride Health will contact the member to schedule their rides. If members need to reach Ride Health during normal business hours, they can call (844) 540-1388.
