Provider Complaints
If any provider has an issue or complaint engaging with our Social Care Network, you may submit a complaint to the New York State Office of Health Insurance Programs (OHIP) or to our Social Care Network directly. If you are a Medicaid member that wishes to report a complaint, please visit statenislandpps.org/member-services.
To submit a complaint to the Social Care Network lead entity, please complete this form. Upon receipt, we will review the complaint. A member of our staff may contact you for more information. Providers will receive a response from the Social Care Network lead entity within 7 business days.
Provider Complaints
To submit a complaint to OHIP:
- Send an email to SDH@health.ny.gov.
- The email should include:
- Name of the organization submitting a complaint
- Name of the Social Care Network lead entity (Staten Island PPS)
- A narrative description of the complaint
- Timeframe of when the event occurred
Upon receipt, OHIP will review the complaint and engage with the Social Care Network to resolve the issue. OHIP may share a response with the stakeholder submitting the response with the resolution or path to resolution, as needed.
